They may look the same, especially for gaikokujin or foreigner eyes, but they are not.
In a nutshell, oirans can be considered as exaggerated geishas. They have more presence and definitely cost more than geishas.
Nevertheless, both kinds of women represent the Japanese culture in very distinct ways.
