What's inside the bag remains a mystery up to this day since most gamers wonder what's inside. If you shoot the bag, it will stop moving. Using an infrared scope, you can reveal that the bag has a heat signature that means there's something living inside. Well, what's inside the bag, no one knows because even the game's wikis across the internet have no definite answer. Probable theories are a child, a body part or a failed regenerator.
